This video from the ATSB forms part of the final report into the collision with terrain involving a 6-seat, Piper PA-32 Lance at Shellharbour Airport, NSW. 

The investigation found that during take-off the horizontal stabilator moved to a full – or near-full – nose up deflection and very likely remained in this position for the entire flight.
